DALLAS NATYALAYA REGISTRATION AND WAVER FORM

Students can enroll during anytime of the year. They would be placed in the appropriate batch based on the available slots.

• Registration fee is $25.00, which is one time. This is collected in order to maintain a separate profile for the student (issue grades and book-keep evaluations).

• Every student (or parent) enrolling in Dallas Natyalaya needs to read and sign the “Registration and Waiver form” or Can register through Online. This ensures that the parent/student is well informed and understand the terms and conditions of Dallas Natyalaya Academy of Dance.

• The registration form can be either downloaded here or taken from the teacher in-person or can be done online.

PROCESS OF SUBMISSION OF FORM

1. Download the Registration & Waiver Form

2. Read and Agree to the Waiver form.

3. Fill in all the details and place a wet signature.

4. Attach a Passport size photograph of the student enrolling (this is for official use only).

5. The registration form can be submitted either by mail or to the teacher in-person at the venue where the classes are conducted. (Please see contact us for more details on the venue)

6. Mail/Submit the completed registration form, along with a check (One time Registration Fee – $25.00) payable to “Sujavanthy Srinivasan”

Note: The late fee ($25.00) is applicable for any delay in Tuition payment.
