TRAIL CLASS FORM
We ask all those who participate in a trial class for any one or multiple classes offered at Dallas Natyalaya Academy of Dance to carefully read and sign below. Without this form signed, you / your child cannot partake in any trial classes
Please provide the following information:

• Dallas Natyalaya Academy of Dance is not responsible for or will be held liable for any injury or medical problem / condition caused during or after the trail class. Students take the trail class at their own risk. Please provide an emergency contact in case a medical problem / injury does occur.
• Dallas Natyalaya Academy of Dance is not responsible for any lost or stolen items.
• Dallas Natyalaya Academy of Dance is not responsible for any damage or injury to a Trial student or a family member or student’s guardian that may occur inside Dallus Natyalaya or in parking lot.
• Please sign and data below in order for you to take trial class
• I agree not to hold Dallus Natyalaya Academy of Dance and its staff liable for any injury or damage to trial student, family member or guardian, if it should arise during or after during or after a Trial Class.
• *This is not the registration form. Once you / your child decide to enroll in the classes, *you will need to complete registration online at www.dallasnatyalaya.com
